Southeast Colorado Hospital District Home Health and Hospice Agency


SECHD Home Health Agency and Baca County Hospice serves all of Baca County, CO and the town of Kim, CO to include an area of one mile radius in Las Animas County, CO. We do not accept patients under the age of 18 years, psychiatric/behavioral health patients, and do not provide care for ventilator patients.
What is Home Health?
Our team is comprised of providers, licensed nurses, physical therapists, occupational therapists, speech therapists, and a licensed social worker. These skilled professionals are committed to providing high quality health care for patients and their families in the home setting.
Our goal is to help coordinate the care and/or therapy your medical provider has ordered. The team works with the patient to provide an individualized plan of care that will be used to help each patient obtain their goals.
Skilled Services
Services offered include management of acute and chronic diseases; medication management and teaching; wound care management to include the use of wound vacs; specimen collection as ordered; management of coumadin therapy; management of diabetes and complications; patient and family education on disease and medication management. Physical therapy establishes a home exercise program focused on strengthening and conditioning, gait improvement and achieving personal goals. Occupational therapy focuses on safety in activities of daily living, and Speech therapy assesses and develops methods to help with cognition, speech disorders, swallowing issues and communication. All these may be provided in the home if ordered by your provider. Services of a licensed social worker are offered in person in the home and via telephone conferencing. All services offered are only done within the scope of the licensed staff available.
What is Hospice?
Hospice is a special health care option that provides patients with a terminal illness the opportunity to live a quality life at home and have as much independence as possible while they are living. Our multidisciplinary team consists of providers, skilled nurses, therapists, social workers, clergy, bereavement counsellors, and volunteers. We are committed to managing the physical, social, emotional, and spiritual needs of each patient and their family at the end of life. Here at Baca County Hospice, we value our patient’s wishes and work to fulfill those wishes. Our team provides compassionate care with dignity, we prioritize pain control and symptom management for all Hospice patients. We also offer counselling for the patient and family, end of life education, caregiver support
and training on how to care for a person at the end of their life.

Cancer Travel Fund:
The cancer travel fund is available to anyone in Baca County diagnosed with cancer and receiving treatment out of town. Funds help with travel and related expenses while the patient is receiving treatment. This travel is funded on donations from the Cancer League of Colorado Service Grant, the Salvation Army, and from individual donations made by community members. Our travel fund is available to anyone in Baca County who has been diagnosed with cancer. Funds are available to help assist with travel related expenses while the patient is receiving treatment.
